Santa Clara Valley Hospice has a mission of continuing commitment of support and care through service, compassion and empathy for those with terminal and life limiting illnesses and their families.
We provide help to avoid hospitalization and also provide physical, psychological and spiritual needs of the family unit. We offer all of our services free of charge and are able to do this through donations from the community. Here is a list of our programs and services we offer the Santa Clara Valley community:
Free Medical Equipment Lending Program
We lend medical equipment free to the Santa Clara River Valley communities. Clients can keep the medical equipment for as long as they need.
Free Medical Supplies Program
We offer free medical supplies for clients including but not limited to:
- Pull-up diapers, wipes and ointments
- Nutrition drinks
- Pads
- Bandages and wound care items
- Oxygen & oxygen supplies
- Skincare supplies

Bilingual Services
All of our services are offered in both Spanish and English to support our diverse community of Santa Clara River Valley.
Community Support
Support groups are a great way to get help in dealing with difficult times. Our support groups meet several times a week providing caregiver training, grief support and bereavement counseling.
Respite Care
We provide respite care services to help relieve caregivers and give them a break. We can come and provide primary care for a few hours so the caregiver can have some time off. We also provide light housekeeping work and decluttering services to help out around the house.

Casework and Advocacy
We provide paperwork assistance with Medicare, MediCal and other healthcare insurance. We also provide advocacy for older adult programs and services that can be helpful in many situations such as navigating the healthcare system, bill paying and statement review, crisis intervention and assisting in doctors visits.

Friendly Visits and Flower Delivery
We offer friendly check-ins and social visits with flower delivery to boost morale and happiness for those experiencing illnesses or loneliness.
